We will email you about your pet’s appointments, vaccination reminders, updated policies, as well inclement weather updates, and estimates for surgical and medical services.

  • Acting in accordance with our core policies – this includes abiding by our physical and psychological health and safety, accessibility, and inclusivity policies.
  • Refraining from disparaging remarks that could be considered harassment - this includes emotional blackmail, swearing, inappropriate name calling or derogatory remarks or gestures.
  • Adherence to our payment policies.
  • That feedback is given in a respectful, constructive way via the available avenues.

Rabies Immunization Policy

I understand that per Illinois State law, county law, and hospital policy, all pets must be vaccinated against rabies regardless of indoor/outdoor status.(Required)

Prescription Refill Policy

FAH requires 24 hours notice for all medication refill requests. Rush same day prescriptions are subject to a rush fee and require at least 2 hrs to fill. Clients are welcome to use a third party pharmacy at their own risk. Any medication filled through a third party pharmacy requires a written prescription signed in ink by the doctor and picked up at the clinic to mail in.(Required)

Financial Policies of Finan Animal Hospital

I understand an estimate of services will be provided to me prior to any services being rendered. Payment is due at the time of service. We accept cash, credit cards, and Care Credit. A booking fee of $89 is collected at the time of booking a surgery or other procedure and $95.00 for an ultrasound. These fees are refundable if the appointment is canceled with at least 24 hours notice.(Required)

Cancellation Policy

Appointments will be seen on time. If you arrive more than 15 minutes past your appointment time you will be asked to reschedule. If you are late, miss your appointment, or cancel with less than 24 hrs notice, 2 or more times, you will be required to prepay for all future appointments or may be dismissed from the practice.(Required)

Behavior Expectations

FAH has a Zero Tolerance policy. Abusive, threatening, or otherwise hostile behavior will not be tolerated. All interactions will be respectful.(Required)

Veterinary Client Patient Relationship

I understand that per the Illinois Veterinary Medical Practice Act, all pets must be examined by a veterinarian within the practice once every 12 months. Medications and food cannot be refilled without a current Veterinary Client Patient relationship.(Required)

Appointment Recording

Our veterinarians and nurses utilize ScribbleVet, a dictation software, which records your pet's appointments for improved clinical documentation. By initialing you acknowledge that your appointments may be recorded.(Required)
